Request your appointment below.  The office must approve your appointment request before appointment is confirmed.  If we have questions about insurance, we will reach out to the phone number provided. 

If your appointment request is denied please call our office for an appointment.

Insurance: We do not accept Medicaid or any Medicaid type insuranceTricare Prime requires a referral.  Other insurance carriers may require a referral. Please check with your insurance carrier to see if you need a referral to be seen. We can not determine if your insurance requires a referral until we see our insurance card.   If you come to your appointment and we then see that your insurance requires a referral, we will not be able to see you until a referral is obtained!

Patient Portal:

You will receive an email with instructions on registering for our patient portal.  Email link expires in 72hrs, if link expires click forgot password to reset login.  Please fill out all sections under "My Health", this will make your check in process quicker.
