Insurance:

We do not accept Medicaid or any Medicaid type insurance.  Tricare Prime requires a referral.  Other insurance carriers may require a referral. Please check with your insurance carrier to see if you need a referral to be seen. We can not determine if your insurance requires a referral until we see our insurance card.   If you come to your appointment and we then see that your insurance requires a referral, we will not be able to see you until a referral is obtained!
